The following forms may be uploaded within the FACTS Application and Enrollment system; but it may be easier to download them, fill them out, and submit to the school office as applicable.
Physical Examination Form, required upon entrance to Kindergarten, 3rd grade, 6th grade, and for all new students
Authorization for Medication Form, if applicable
Authorization for Over-the-Counter Medications, if applicable

Application & Enrollment
For Current Families
Re-enrollment for current students for the 2024-2025 school year is underway. The enrollment process will continue to be managed via the FACTS Application and Enrollment program.
For New Families
Applications for new students for the 2024-2025 academic year are being accepted.
General Admissions Information
Students applying to Kindergarten are asked to complete a learning readiness evaluation conducted by one of our teachers. The school will contact the family to set up an appointment for this evaluation.
Students applying to Grade 5-8 are asked to complete an admissions interview with the Principal and or a grade teacher. The parents are invited to the interview as well. The school will contact the family for a mutually agreeable date.

It is understood that our program is not appropriate for every child and that the Admissions team is not only deciding what is best for the school, but also what is best for the child.
Admissions Priority
Students fulfilling the above criteria will be accepted based on availability of openings and in the following order of priority:
- Siblings of current Saint Joseph Students or Alumni
- Parishioners of St. Joseph
- Non-parishioners and non-Catholics
